Scheduling an Appointment
To schedule, reschedule, or cancel an appointment at any of our locations, please call our main phone number during phone hours. When calling to schedule, it’s helpful to have your insurance card handy. New patients are welcome — please also see our New Patient Registration page for the forms to complete before your first visit.
Medical Records and Referrals
Requests for medical records may be faxed to (916) 486-0946. If you are transferring your care to Camellia Women’s Health, our Medical Records Release form allows us to request records from your previous provider on your behalf.
Insurance Questions
We accept most major insurance plans. Please visit our Accepted Insurances page for a list of carriers, or call our office if you don’t see your plan listed.
