To help make your first visit as smooth as possible, please print each of the following forms, fill in the blanks to the best of your ability, and bring them to your appointment for review with the physician, physician assistant, or nurse practitioner.

• Medical Records Release

Completing these forms ahead of time will speed up your registration and reduce your wait time on the day of your appointment.
 

What to Bring to Your First Appointment

In addition to your completed forms, please bring the following with you:

• A current photo ID
• Your insurance card
• A list of any medications you are currently taking, including dosages
• Records from a previous provider, if applicable (the Medical Records Release form above allows us to request these on your behalf)

 

Your First Visit

At your first appointment, your provider will review your medical history and forms with you and discuss your health needs and any questions you may have. At Camellia Women’s Health, we believe you should feel educated and empowered to make decisions about your own health — so please don’t hesitate to ask questions at any point during your visit.

We accept most major insurance plans and are currently welcoming new patients at our Sacramento-area locations in Downtown Sacramento, Granite Bay/Roseville, and Carmichael.
